The AHTN mission is to coordinate a sustainable provider network of care for low-income uninsured residents of Berkeley, Charleston and Dorchester counties.

AHTN is a community program that provides care coordination to low income individuals not covered by health insurance and who do not have a doctor or medical home.  By connecting the Lowcountry’s low income, uninsured residents to a medical home, we are able to better meet their primary care needs and, in turn, improve the health of our community.
